Start Therapy with Us

We are currently accepting new clients! To protect your privacy and ensure your

information is securely uploaded straight into our medical records system, we use a simple, two-step intake process.

Step 1: Submit Your Request

Fill out the brief contact form below with your name, contact information, and a brief message.

Step 2: Complete Your Intake Packet

Once we receive your message, our team will create your secure client profile in our electronic health record system.

  1. Within one business day, check your email for an invitation link from TherapyPortal/TherapyNotes.

  2. Click the secure activation link in that email to open your private portal.

  3. Complete and digitally sign your intake packet right there on your phone or computer.

Once your paperwork is submitted through the portal, we will contact you within 2 business days to schedule your first appointment!
